Chronic care for PDOC patients

If the patient has not yet regained consciousness after the PIN program, he/she will continue to the chronic care program in consultation. Together with healthcare professionals, they will look at the most appropriate care after intensive neurorehabilitation.

What is the chronic care program?

The chronic care programme will be initiated after the treatment objectives for the EIN and the PIN have not been achieved. The patient has not regained consciousness. During this phase, the healthcare professionals talk to the patient's relatives. The follow-up process will be discussed in this meeting. It is determined whether there will be a follow-up in chronic care or palliative care. Chronic care is focused on quality of life, while palliative care is focused on end-of-life care.

Who is the chronic care program for?

The chronic care program is intended for people who have not regained consciousness during intensive neurorehabilitation (EIN and PIN).
